Home health care is a service provided
to people who have a medical need which requires
the skills of a nurse or therapist.
Most patients who use home health are those who
have recently been hospitalized and have a change
in their condition or medicines, a new diagnosis,
or those who have had surgery.
Many people who require dressing changes can
also benefit from home health. Home health is
provided to all ages of individuals from infants
to the elderly.

- You will qualify for Home Health if you...
- can be safely cared for in your home or the
home of a family member.
- have a skilled need as a result of injury,
illness or surgery that requires the care of
a registered nurse or rehabilitation therapist.
- have a need that can be provided on an intermittent
basis by home health and if it cannot be, have
family/caregivers who can be taught to provide
the care.
- have home health ordered by your physician.
